This Notice describes how information about you that may constitute protected health information may be used and disclosed, and how you can access this information. Please review it carefully.
PAWsynergy Health & Fitness LLC ("PAWsynergy," "we," "our," or "us") provides non-medical in-home pet wellness services. Veterinary telehealth services for your pet are provided by Go Lexi through the MyPrimaryCare platform, operated by Strategic Investment Partners Inc. d/b/a MyPrimaryCare. Where information you provide about yourself is used to coordinate human-facing healthcare access through MyPrimaryCare affiliates, we and our partners commit to handling that information in accordance with the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and applicable federal and state laws.
Scope
Most information PAWsynergy collects relates to pets and is not protected by HIPAA. This Notice applies to any individually identifiable health information about a human member that we or our partners receive in connection with the membership.
Our Responsibilities
- Maintain the privacy and security of protected health information
- Provide this Notice explaining our legal duties and privacy practices
- Follow the terms of the Notice currently in effect
- Notify you promptly if a breach occurs that may have compromised the privacy or security of your information
How We May Use and Disclose Information
Treatment
We may share health information with veterinary and healthcare providers involved in care coordination, including Go Lexi telehealth providers and affiliated medical networks.
Payment
We may use and disclose information to bill and collect payment for services through our Platform Partner and payment processor.
Healthcare Operations
We may use information to operate and improve our services, including quality improvement, administrative functions, and compliance activities.
Other Permitted Disclosures
- As required by federal or state law
- For public health activities
- To comply with court orders, subpoenas, or legal proceedings
- For health oversight activities
- To avert serious threats to health or safety
- To law enforcement when permitted by law
Business Associates
We may share protected health information with trusted partners who perform services on our behalf, including Strategic Investment Partners Inc. d/b/a MyPrimaryCare (platform, scheduling, payments), Go Lexi telehealth providers, and technology service providers. These partners are required by law and contract to protect your information.
Your Rights

You may request access to or copies of your health information.

You may request that inaccurate information in your records be corrected.

You may request limits on how we use or disclose your information.

We reserve the right to update this Notice at any time. Updated versions will be posted on our website with a revised effective date.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
Arbitration and Governing Law
Any dispute, claim, or controversy arising out of or relating to this Notice or the services shall be resolved exclusively through binding arbitration in the State of Indiana, and governed by Indiana law. You waive any right to participate in class actions or jury trials related to such disputes.
